Hi Scott,
Bleed value is generally equal on all sides. Generally 1/8 inch is fine
unless your printer specifically asks for more. You didn't mention the
crop marks. No mention of the register marks as well. It may be a good
idea to check with your printer what's needed. If you are going to put
the crop marks (some printers will definitely ask for that) you might
consider making your whole document a full inch wider and longer than
the final page so you have space for bleed and for the marks.
