I don't have an article but I have published an e-book which costs
$5.95 (see below). I also have my own page dimensions calculator which
gives you the dimensions and guide locations of a perfect bound book.
All you need are the finished size, the page count, and the PPI (pages
per inch) for the particular paper stock. Please visit:

and fill out the blanks. 

For LSI the white paper stock is 534 PPI and the creme is 459 PPI.
The extra allowance for the thickness of the cover itself is .023 inch
(the e-book states .022 inch but I find .023 inch is a hair more
accurate.)

Since you have both 1.4.0 and 1.5.0 I suggest you design the cover in
1.4.0 and then at the end load the sla file into 1.5.0 to save as pdf
X/1-a:2001 format. Or you can save as tiff from either version.
