1. It's worth a try to just put a complete URL in your document. Adobe 
Reader usually converts this to a link.

2. Otherwise, it's a bit annoying. You can make a small frame as Helio 
mentions above, have it link (Context menu > PDF Options > Annotation 
Properties) and enter your information for the link.
The problem is, this will not have any content in the PDF, it will just 
be an area where the mouse is active, so you'll need to place this above 
your TOC frame where you want someone to click.
