Sooner or later, you have to decide some very basic things like page 
size, what sort of layout the text will have (more than one column or 
not, what sort of margins it will have), something about the size and 
type of font you are going to use. You should typically be talking about 
a double-sided layout of pages, of course.

You can play with these with a fragment of your book. Once you get 
settled on these basics, then you can construct a layout of the right 
size with automatic text frames. This way, when you add pages, they will 
already have your frames in place, ready to import the text and 
automatically be linked. You should make some Paragraph/Character Styles 
to work with, realizing you can tweak these later. The Master Pages are 
typically essential, yet probably not one of the basic things to worry 
about too much at the beginning.
